Enabling Patient-Centered Medical Homes
When Alice Jackson*retired from teaching school in rural North Carolina, she found herself isolated and in failing health. In her early 60s, she’d been independent all her life and didn’t want to ask anyone for help. Her eye-sight failing and other health issues going untreated, Jackson feared that instead of being self-sufficient and living in her own house, she’d end up spending the rest of her days in a nursing home. But thanks to an innovative health management strategy by the North Carolina State Health Plan (NCSHP) for Teachers and State Employees, Alice Jackson and thousands of others with serious chronic illnesses will receive the help they need to take control of their health and their lives. In collaboration with ActiveHealth Management and a growing number of physician practices, NCSHP is supporting Patient-Centered Medical Homes (PCMH) across the state to improve the health of Plan members.
